registrationNew Patient Registration

You will need to collect a registration pack from our reception. This will include the necessary form to fill in and return to the surgery. There will also be a short questionnaire to fill in to provide the practice with other important information, and you will be asked to attend the practice for a New Patient Check while we await your notes arriving from your previous doctor. You will be allocated to a doctor’s list, but you may ask to see any doctor of your choice when making an appointment.

It can be very useful to us to know your mobile telephone number as well as land-lines. If you would like us to contact you by text or e-mail, please give us your number/address.

Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
